Membership Application
This form required to become a member of SPP. You can fill this form in online, then print and send it by regular mail using the postage paid label above.
Contribution Form (44Kb)
This form is used when you make your SPP contribution. If you download this form, it must be mailed directly to SPP. This form can be used to contribute to any open plan year.
PAC Application (745Kb)
This is the form you will need to complete if you wish to make your contributions directly from your bank account using the Pre-Authorized Contribution system.
PCC Application (407Kb)
This is the form you will need to complete if you wish to make your contributions with your credit card on a pre-authorized arrangement.
Designation of Beneficiary form
This is the form you will need to complete if you wish to change the beneficiary of your SPP account. You can fill this form in online and then print and mail it to SPP using the postage paid label at the top of this page.
Direct Deposit Request form (739KB)
This is the form you need to complete if you wish to change the account that your pension payment is deposited to.
Application for Death Benefits form (501KB)
This is the form you need to complete if you are applying for death benfits from an SPP account.
Voluntary Income Tax Deduction form (95KB)
This is the form you need to complete if you want SPP to deduct income tax from your pension payments.
Transfer and investment instructions form (95KB)
This is the form you need to complete if you want transfer funds between investment options available from SPP.
